Looking at the maiming houserule: I think it's a good idea, but blinded (for example) is a bit steep of a penalty to have until the next extended rest. Maybe take out one eye at a time? (e.g. Penalty to perception checks and provide combat advantage)
Also, the maiming rules definitely disadvantage PCs more than monsters. As it is written now, I doubt we'd end a day without someone maimed.

Good point on the blinded penalty. I'll take your suggestion for perception checks. Instead of combat advantage, how about "dazed until end of next turn. Aftereffect: -4 penalty to Will"?
Is it more disadvantageous for PCs? Yes, but you kill monsters. I'd say being dead is a pretty big disadvantage. I came up with the idea thinking it would make things a bit more difficult in an interesting way.
Also, the chance of actually getting maimed is only 20%. I could lower that to 10% if that seems more fair. Ending the day with a maimed character is OK. If you have magical healing during an extended or wilderness rest, the maiming can be healed.

Suggestion: what if instead of "left arm" and "right arm" you made it "main hand arm" and "off hand arm" and had different penalties for each? [e.g. you can't use a shield with that arm (or your shield bonus is reduced), you take a penalty to attacks with weapons in that hand] That may allow for a greater variety of penalties.

Wilderness rest and maiming are pretty harsh houserules. What if on turn 1 on encounter 1 you maim someone and hit their arm? -4 penalty to attack rolls for the rest of the day? There goes that character's ability to play for the rest of the day. Remember our base attack for a non power started character ranges from 6 to 9 at this level. I would vote that we have an immediate effect till end of encounter and a permanent effect till end of day. Also I believe you should roll a d4 instead of a d6 that way It's a even chance. End of encounter effects should be the penalty to player ability. End of day effect should be penalty to defense. Legs are reflex. Arms are ac. Etc.
Now for wilderness rest the thing that disturbs me most is the fact that you can only regain surges equal to your con mod. That completely trashes some characters and isn't so bad for others. At worst someone can gain 1 surge. At best someone can gain 7. Why not make it just half of your surges? That way it is still based on con mod but only half as deadly.

Re: wilderness rest. I do think this one is harsh. I was actually thinking of making it something like 2 + con mod+ mile stones for surges. I'll ask at the table to see what seems more fair. We might not even implement it.
Re: maiming. A round 1 turn 1 maiming is unlikely to happen. Firstly a critical hit is a 1 in 20 chance. Then the maiming is 1 in 5 on top of that. So that's 1 in 100 chance. I said before that I'll consider reducing it to a roll of 10%. Hell, I'll consider a roll of 5%. So that would be what, 1 in 400? Lastly: DM discretion.
Thanks for the idea about the immediate effect until end of encounter and permanent effect until end of day. That makes mechanical sense, I think. We can discuss more at the table.
